Transcripts & Enrollment Verification

Transcripts are provided free of charge to current enrolled students. Please stop by the Office of Student Services or email Maribel Cornejo to submit your request.

Alumni Requesting Transcripts

Online payment for ordering of transcripts is not available at this time. Transcripts currently cost $10 per copy, payable by check, money order, or cashier’s check to “UC Regents”.

Submitting Request

You may submit your request in person, by mail, fax, or e-mail.

In your request, include the following:
  • Your name when you graduated from UCLA Dental School

  • Your dates of attendance (year of graduation)

  • Your last 4 digit of your Social Security number

  • Your contact telephone number

  • The address to which the transcript should be sent

Processing Time

Whether you place an order in person, by mail or email, please allow 5-7 business days for processing from the date received by the Office of Student Services – Registrar.

The Office of Student Services – Registrar prints transcripts on special security paper, seals the transcript in an envelope, and mails the transcript to the address that you specify. You also can choose to pick up the transcript in person at the Office of Student Services Suite A0-111 (in the School of Dentistry, Center for the Health Sciences). Please be sure to have a valid ID, i.e. driver’s license, state-issued identification card, or passport. 

The Office of Student Services – Registrar does not directly send electronic PDF transcripts to recipients at this time.
